Patient Portal

Thank you for using our Patient Portal.

Entering the information from home before you come in will save time when you’re in our office.  Later, you’ll be able to modify information and retrieve summaries of your visits.

Through this portal you will be able to directly enter and change information we use for your visits.

Please note:  For technical reasons, the following information can only be changed by our staff:

  • Name
  • Social Security number
  • Phone numbers
  • Email addresses
  • Address
  • Employment information
  • Insurance

You can modify:

  • Your pharmacy
  • Your medical history
  • Your skin disease history
  • Your allergies
  •       and much more.


You don’t need to enter information for everything: not all fields pertain to every patient.  Please be thorough for the information we will need to treat you properly.

If you’re a new patient, a Username and Password can be given to you when you call for an appointment.  Existing patients are given this information face to face in the office.

After you’ve been seen in our office, we will have your visit information available to you online.  Because we have to protect that information, we will not be able to give out your password over the telephone.  If you lost your password, we will be happy to reset it for you when you come in.
